The responsibility falls on leadership of the potential incident, However the Air Force is not the only service with issues. The radiation leaks in the Japanese port and also a Nuclear sub wher check list procedures were over looked and by bassed bothe were posted on Military.com and in the news around the same timeframe. I’d venture to say the spotlight on this incident caushioned other leaders to tighen up before they had issues also.
